TERMS AND WARRANTY CONDITIONS:

INSURANCE RENTAL POLICIES AND PROCEDURES

  • Lessee must complete Insurance Information Packet.
  • Lessee must bring their Insurance Health Plan Card and a picture ID to prove eligibility.
  • Lessee must have a prescription for the breast pump rental that includes:

    o Physician’s printed name (circled if there are multiple names) 

    o Physician’s signature

    o Physician’s License Number(NOT DEA# or NPI#) 

    o Mother’s name

    o Diagnosis (medical reason the pump is needed) and a Time Frame (how long you may need the pump)

    o ANTHEM BCBS/Humana Rentals require “Hospital-Grade” on the prescription

  • Lessee must contact Babyology at the beginning of each month to renew the rental.
  • Lessee must notify Babyology of any address or phone number changes.
  • If you fail to contact Babyology about changes in your contact information, or you are no longer

    using the pump for the reasons it was prescribed, you may become responsible for the bill.

    When your eligibility expires you must return the pump to the Babyology office or you become responsible for the pump rental fees.

    You must provide any other insurance coverage information to Babyology.

    (Medicaid) If your Medicaid status says Presumptive Eligibility your insurance will not cover the pump rental.

    (Medicaid) All other insurance is primary and must be filed prior to Medicaid.

    If you do not provide the information necessary to file your insurance, you are responsible for payment.

    If your insurance requires pre-authorization, it is your responsibility to provide us with this information.

    When you stop using the pump, you must return it to the Babyology office. If you fail to return the pump when you are finished using it, YOU will be responsible for all rental fees from that day until the date of return, as per signed rental agreement. 

    RENTAL EQUIPMENT WARRANTY INFORMATION
    Rental/Purchase DME Equipment
    All rental pumps rented from Babyology have a manufacturer’s warranty. Babyology will notify all customers of the warranty coverage, and we will honor all warranties under applicable law. If at any time a RENTAL pump stops working, Babyology will replace it free of charge and send it back to the manufacturer.
    Babyology will notify all customers of the warranty on any purchased DME equipment. Babyology will also provide an owner’s manual with warranty information where this manual is available. 



    INSURANCE PURCHASE POLICY

    If, for any reason, your insurance does not pay for your breast pump, you are responsible for the portion they do not cover.  Before you receive your breast pump, you are required to have a credit card on file, for any unpaid balance.  You will be notified, if there are any charges made to your credit card.

    PURCHASE EQUIPMENT WARRANTY INFORMATION

    Purchase of a Single User (non DME) Equipment
    All purchase pumps purchased from Babyology are covered under manufacturers warranty. The warranty on the purchase pumps is one year on the pump mechanism and 90 days on all other parts from the date of purchase.
    The warranty is honored through the manufacturer NOT Babyology.       

    RETURNS:

    We accept returns on a limited basis as a service to our customers.
    All returns
    - must be accompanied by a receipt,
    - must meet the conditions outlined below, and
    - must have an Return Merchandise Authorization (RMA) obtained by salesinfo@babyology.com with your name, order number, item you would like to return, and reason for return.
    Packages received without an RMA will be refused and no credit will be given.
    Shipping costs are not refundable.

    If non-qualified products are returned to Babyology, they will not be refunded or returned. No returns on clearance merchandise (which includes anything being sold for over 25% off), "While they last" items, items marked special order, breast pumps, scales, and other products specified below.

    Bras and Clothing: Unless otherwise stated in the product description or indicated below, if clean, unwashed and unworn with tags attached and complete packaging, bras and clothing can be returned within 30 days of your invoice for a full refund. Clearance (which includes anything being sold for over 25% off), "While they last", and special order items are not returnable for any reason.

    Defective bras or clothing: All defective merchandise must be reported within 30 days. In most cases, we will give you information to directly contact the manufacturer to deal with the defective item.

    Swimsuits, hosiery and support belts (maternity or postpartum) are not returnable for any reason for hygienic reasons.

    Breast pumps: Breast pumps are NOT returnable for any reason for hygienic reasons. Breast pumps are personal use products and we want to be sure you are getting the best for your family. Unless we specifically state otherwise, all of our breast pumps are brand new, have the original manufacturer’s seal and are under full manufacturer’s warranty. 

    Breastfeeding products (breast pumps, parts, special feeders, pads, bottles...): Unless otherwise stated in the product description, if unopened and in brand new condition, we will accept returns within 30 days of purchase. If non-qualified products are returned to Babyology, they will not be refunded or returned. Since breastfeeding products are personal items, we want to ensure all of our customers that the products are brand new. Unless we specifically state otherwise, all of our breast pumps are brand new, have the original manufacturer’s seal and are under full manufacturer’s warranty. All of our breastfeeding supplies are brand new.

    Clearance (which includes anything being sold for over 25% off), "While the last" and items marked special order in the description are not returnable.

    Food is not returnable for any reason.

    Return method: We suggest returning your package to us insured, signature required. We are not responsible for damaged, lost or misdelivered items. A refund will be issued to the same card on which the purchase was made within 3 business days of us receiving the item in acceptable condition with the proper RMA. If purchase was made using a gift certificate, a new gift certificate will be issued.
